The head of the Tennessee prison system resigned after a failed execution.

A former US attorney, Ed Stanton, was appointed to lead an independent review of the situation. 

Tennessee Department of Correction Commissioner Frank Strada is set to leave his post this month, Gov. Bill Lee said on Saturday.

Christa Pike was set to be executed for murder.
